Breakthrough partners with those affected by poverty to build connections, develop skills, and open doors of opportunity.

Breakthrough Behavioral Health provides a comprehensive and integrated approach to health care including medical, mental health, behavioral, and dental care. The program increases access to health care for a vulnerable population, while improving health literacy, decreasing care fragmentation, and addressing behavioral and social determinants of health. The program operates out of a trauma-informed, housing first, harm reduction model of care that puts client choice at the center and increases the possibility of long-term stability. Behavioral Health services are offered within two transitional housing shelters. The shelters house 30 men and 30 women who complete a program designed for rapid re-housing within 120 days. Each guest meets with a Licensed Clinical Social Worker to develop a treatment plan which can include on-site psychotherapy, addiction treatment, 1:1 coaching, and group therapy. Originally offered to Transitional Housing guests only, the Behavioral Health program has grown to include guests in Breakthrough’s Daytime Support Centers which are open from 9 a.m. to 3 p.m. daily to provide basic needs such as food, showers, laundry, and access to technology. Breakthrough employs two Wellness Coaches who meet with guests and conduct needs assessments. Wellness Coaches coordinate services for adults in crisis, including mental health services, social services, medical services, emergency shelter, and other resources.

What would a $1,000 contribution to your program / organization provide?

$1000 would support two weeks of behavioral health needs assessments and care coordination at one of Breakthrough’s shelter facilities.

Give Time, Things, Support

Give Time (volunteer opportunities) in July
Breakthrough’s Fresh Market food pantry welcomes volunteers to help unload deliveries, organize groceries and household essentials, and package up food for families. Volunteers may also help with food distribution at the Fresh Market and door-to-door delivery in the local community.
